Web Form Input to PDF

I was looking to create a web form that would directly take the inputs and put them into a pdf that some one could immediately print out. Does anyone know how to do this?

It will be possible using PHPlib once we have access to a PHP server.
Right now, you'd have to run your own server, which you may be doing in any case to handle your forms processing.
