There's a form example, but it doesn't function when pasted into a webpage. How do you complete the form so it emails results?

There's a form example, but it doesn't function when pasted into a webpage.

The example is a UI element, and shows how to use the latest CSS/JS styles. You still need to hook it up to some sort of action (by using the action attribute in the form tag):

How do you complete the form so it emails results?

That depends on what you hook the form up to. If you're on Frontier, I believe they have a form parse to email script:

Otherwise, here is a PHP script:

The form also has 'required' fields indicated, but there's no validation javascript. How do you get the fields labeled, "required" so they actually are required?
